KVKK APPLICATION FORM

Under Law No. 6698 on the Protection of Personal Data (KVKK), certain rights regarding the processing of personal data are granted to data subjects defined as "Relevant Person" in Article 11 of the Law entitled "Rights of the Relevant Person". In this context, the Relevant Person must submit applications to our Company as Data Controller to exercise these rights in accordance with Article 13 of the KVKK, in writing or by other methods to be determined by the Personal Data Protection Board.

Applications to be made to our Company in writing must be submitted using this form:

By registered mail with return receipt,
Through a notary,
By signing this form with a "secure electronic signature" as defined in the Electronic Signature Law No. 5070 and sending it to our Company's registered electronic mail address.

Our Company will finalize requests regarding the exercise of rights under Article 13 of the Law free of charge as soon as possible and within thirty days at the latest from the date the request reaches us. However, if the process requires additional cost, our Company may request the fees determined by the Board from the data subject making the application.

Our Company may accept or reject the requests directed to it by explaining the reasons, and will notify the person making the request of this response in writing or electronically.

If the information and documents provided by the data subject to our Company are incomplete or incomprehensible, our Company may request information/documents for the purpose of clarifying the application, determining whether the person is the actual owner of the personal data subject to the application, or ensuring data security; and may direct additional question(s) to the personal data subject regarding their application.

This application form has been prepared so that we can respond to your application correctly and within the legal period by determining your relationship with our Company and, if any, fully identifying your personal data processed by our Company. Our Company reserves the right to request additional documents and information (such as copy of identity card or driver's license) for identity and authority verification for the purpose of eliminating legal risks that may arise from unlawful and unjust data sharing and especially ensuring the security of your personal data. If the information regarding your requests transmitted within the scope of the form is not accurate and up-to-date or if an unauthorized application is made, our Company does not accept liability for requests arising from such incorrect information or unauthorized application. All liability arising from unlawful, misleading or incorrect applications belongs to you.
